Customer Commissioning Form

Installers are to please complete the following form for each new Air-Fibre installation they do.

Once this form is completed, the customer will receive an e-mail from our ticket system which will include:

  1. Standard Venture-Net Support policies and processes.
  2. Customer Information form which needs to be completed and e-mailed back to Venture-Net.
  3. Debt order Mandate for completion by the customer.

It is important for the installer to take a screenshot of the signal strength screen from WinBox, an example of which can be found below. Please select the screenshot file to upload as proof that the customers signal strength was sufficient at the time the service was installed.
